Wrap Around Services for COVID Relief

The Community Action Agency of New Haven (CAANH) utilizes Community Health Workers (CHW) to assess clients facing socioeconomic barriers in CAANH’s catchment area. They provide resources to enroll people experiencing barriers to obtaining and maintaining housing, food insecurities, and negative financial impacts into public and private assistance programs. CAANH provides needed wrap-around services to support healthy living, such as connecting people to healthcare, including the COVID-19 vaccine (making appointments, arranging transportation, etc.). CHWs assess clients in achieving improved health outcomes during case management services.

Individuals or families who have a COVID impact and are within the program eligibility guidelines may qualify for the following supports:

  • Eviction prevention
  • Up to two months of back rent payments
  • Security deposit
  • Transportation
  • Applying for DSS services (i.e. Benefit coordination)
  • Access to services
  • Basic needs
